[Hancock, MI] The Portage Health Foundation, a non-profit charitable foundation serving Baraga, Houghton, Keweenaw and Ontonagon counties, is pleased to announce it is now accepting grant requests to address Community Health, Wellness and Prevention services.
“Over the past several months, the PHFoundation has been working with various community stakeholders to identify and assess community need.” said Kevin Store, Executive Director of the Portage Health Foundation. As a result, the Portage Health Foundation has announced an initial allocation of $150,000.00 to be awarded.
“This is just the beginning,” added Lennon Mattila Hughes, Chair of the foundation’s Board of Directors. “We recognize the incredible need that exists in our community and we are working on ways to begin reinvesting more money back into our communities.”
Grant requests of all scope and size, up to the stated initial allocation will be considered as part of this RFP notification.
Grants addressing the following will be considered under this Request for Proposal (RFP):
1. Initiatives that promote and/or demonstrate collaboration among charitable non-profit service providers to enhance the provision of health and wellness services to the citizens of the stated service territory.
2. Community Health and Preventive Health initiatives that address identified areas of critical health risks in the community including:
• Child and Adolescent Health
• Infectious Disease
• Chronic Disease and Mortality
• Substance Abuse and Addiction
• Behavioral and Mental Health
• Geriatric Health and Nutrition
